What is the role of the ACS Health and Social Service Consultant?
ACS Consultants help both programs and consumers in the health and social service industry to maximize efficiency and effectiveness for the best client outcomes. ACS Consultation is provided by a Social Worker with a Master’s in Health Administration and more than 30 years of experience within the health and social service industries. As a client, your consultant can help you and your family or other representatives you choose to better identify your needs and options for meeting your needs.
Who does ACS help?
ACS provides Geriatric Care Management for all older adults, and specializes in working with individuals with Alzheimer’s Disease and related dementias. In addition, ACS serves Adults with Disabilities and Transition Age Youth (high school students who are planning for or are in the process of transitioning into independent adulthood or into the adult service systems). We also provide assistance and consultation for family and friends of individuals who may be in need of care and are unsure how to approach the topic or need assistance.
How much does your service cost?
ACS wants to ensure that all who need consultation receive some assistance. If cost is a concern, this can be discussed in your initial FREE phone consultation.
Do I need to schedule an appointment?
For your convenience, your initial consultation will take place immediately when you call if a consultant is available. If not, your call will be returned no later than two business days.
Where do we meet?
ACS may meet with clients, their family members, and others, as needed, and at your direction by phone, by video conference, or in-person at your home, a facility or in any other location that best meets your needs. This will be discussed during your initial phone consultation.
