Renew, Refresh, Rebuild Your Care Plan

Welcome to Access Care Services (ACS)

Welcome — we’re truly glad you’re here.

At Access Care Services (ACS), our heart is to help people find clarity, confidence, and practical support as they navigate real-life health, aging, disability, caregiving, and life transitions. Whether you’re facing a new diagnosis, trying to make sense of services and benefits, supporting a loved one, or simply wanting to plan ahead—ACS exists to help you learn your options, lead your care, and move forward with a steady plan.

Who ACS is here to serve

ACS was created for you—especially if you are:

  • An older adult who wants to stay independent, safe, and well-supported

  • An adult living with a disability who needs tools, resources, and advocacy to live fully and with confidence

  • A transition-age youth/young adult learning how to manage healthcare, benefits, education/employment supports, and adult responsibilities

  • A caregiver (family, friend, or neighbor) who wants guidance, encouragement, and practical steps without feeling overwhelmed

  • A health or social service professional who supports individuals and families in the community and wants reliable, organized education, tools, and consultation support

We know the care journey can be confusing, emotional, and time-consuming. It can also be filled with deep meaning, growth, and connection. ACS exists to support both realities—with resources that empower you and with services that support you when you need more than information.

The Golden Guidance Resource Center was built for you

ACS is proud to offer the Golden Guidance Resource Center—a growing, organized library of educational articles, checklists, planning tools, and practical guidance.

Golden Guidance was created to be a very cost-effective way for individuals, families, and professionals to access high-quality information and direction—without having to “figure everything out the hard way.” Our goal is to help you reduce stress, avoid preventable crises, and make decisions with more clarity and confidence.

At the same time, we know that information alone isn’t always enough. That’s why ACS also offers individualized consultation and support for people who want help applying guidance to their unique situation.

How to use the ACS website

We designed this site to be practical and easy to use—because when people are stressed, they don’t need more confusion. Here are a few helpful starting points:

  1. Start with the Golden Guidance Resource Center
You can begin leading your care better with some of the Free Resources provided. However, when you subscribe, you are able to browse by category or topic (aging, disability supports, caregiving, mental wellness, financial wellness/benefits, planning ahead, living well, and more). If you’re not sure where to begin, start with the category or topic that best matches your current interest for learning.

  2. Use the search feature
If you have a specific question (transportation, home care, long-term care planning, advance directives, guardianship alternatives, caregiver burnout, etc.), use the search tool to quickly find related resources.

  3. Look for checklists and action steps
Many articles include practical “what to do next” steps, conversation guides, and downloadable tools to help you take action.

  4. Know when to reach out for personalized support
If your situation feels more complex—or if you want a second set of trained eyes to help you organize options, plan next steps, or communicate with providers—ACS consultation can help.

Golden Guidance is your starting place and may be all you need — However, ACS is here if or when you want more support.

Golden Guidance is designed to help you build understanding and confidence. But some situations require individualized guidance, such as:

  • coordinating services across multiple providers

  • navigating benefits and eligibility questions

  • caregiving in a crisis or during rapid decline

  • preparing for hospital discharge or a move to assisted living/skilled care

  • advocacy concerns and quality-of-care oversight

  • planning for long-term needs and reducing “unknowns”

When needed, ACS is here to walk with you—step-by-step—to build a plan that fits your goals, values, and real-life circumstances.

A meaningful way to support a loved one

Sometimes the best gift is not another item—it’s clarity and peace of mind.

You can support a loved one by purchasing a gift certificate for consultation, which can be used for:

  • Education specific to your needs
  • planning and organizing care needs

  • caregiver coaching and support

  • resource navigation and referrals

  • preparing for major transitions

  • creating a personalized action plan

It’s a thoughtful way to say: “You don’t have to figure this out alone.”

We welcome your feedback and ideas

ACS is committed to continuous improvement. We want this website to be:

  • easy to navigate

  • clear and practical

  • relevant to real needs

  • encouraging and empowering

If there’s something you wish you could find, a topic you want us to cover, or a way we can make the site more user-friendly, we truly welcome your input. Your feedback helps us build a resource center that serves people better—one article, one tool, and one improvement at a time.

Our purpose: equip, empower, and encourage

At ACS, we believe people deserve support that is:

  • respectful and person-centered

  • practical and organized

  • grounded in dignity, compassion, and hope

We want to help individuals live the best lives possible, help caregivers feel confident and supported, and help professionals feel equipped as they serve people across our communities. Caregiving and care-receiving can be difficult—but there are also blessings along the way: deeper relationships, meaningful moments, growth, gratitude, and the comfort of not being alone.

ACS aims to be your go-to support—your “starting point” for clarity and your steady partner when you need more help.

Again, welcome. We’re honored you’re here, and we’re ready to serve you.

Learn your options. Lead your care.
