This series has been designed for Continuing Education to prepare Social Workers and other Health and Social Service professionals in the knowledge, skills, and confidence necessary to equip and assist decisional adults, from High School Seniors to Senior Citizens, with advance care planning and care management tools and skills to help them be leaders of their own care.
